Potency Examination & Certification

POTENCY in a male is defined as the “the ability or capacity to achieve an erection which is sufficient enough to have satisfactory penetrative intercourse”

A male is said to be IMPOTENT when there is inability to develop or maintain a penile erection sufficient to conclude the act of intercourse to orgasm. 

Impotent person can achieve fertility if he has a normally functioning testis with adequate spermatogenesis via Intra- uterine Insemination, Testicular sperm aspiration and IVF techniques.

Potent erection is not necessary for ejaculation and Patient can be potent despite being infertile or Impotent despite being fertile

Testing will involve 4 steps :   (may require more than one session) 

  • History taking

  • Office Sildenafil Test and/or Intracavernosal Injection of Vasoactive drug (ICIVAD)

  • Investigations

  • Certification

Blood test - commonly required

(a) Serum testosterone and sex hormone binding globulin- to exclude endocrine causes.

(b) Fasting glucose and glycated hemoglobin (HbA1c)- to exclude latent diabetes mellitus.

(c) Lipid profile- for deranged lipid profile.

(d) Thyroid hormones- for hypogonadal patients.

(e) FSH- for patients with special clinical conditions.

Report provided will be an extensive 5 page report with the conclusion as follows :

There is nothing to suggest at the time of examination that the person examined is incapable of performing sexual intercourse under normal circumstances ”.

The Clinical  examination and testing suggest that the person  is incapable of performing sexual intercourse at the time of examination and that the  impotence is due to   ___________(etiology) and likely to be reversible/Irreversible .”
